Today, we find that for patients to recover faster and improve their health, it is essential that the clinician has clear and accurate pictures of how the patients are feeling, how they are limited because of their health, how they are suffering, and what their expectations are of their doctors. In addition, we want our doctors not only to reduce the burden of illness but also to help the patient avoid illness, prevent disease, and promote health. Therefore, we want to make it easier for patients to partner with their health teams to avoid illness and injury, and to make it possible to live longer and healthier lives. This requires good communication. We have seen that it is possible by transforming the voice of the patient into information that is meaningful and actionable.
