When things go wrong at emergency scenes the casualty investigators often cite flaws in situational awareness. These findings have only limited value. What would be of greater benefit is for first responders to understand HOW and WHY situational awareness is flawed and how to ensure the ability to develop it, maintain it and if lost, regain it.
