The captain, copilot, and eight passengers aboard the helicopter were killed, and the helicopter was destroyed by impact forces. The flight was operating under the provisions of 14 Code of Federal Regulations Part 135 on a visual flight rules flight plan. Night visual meteorological conditions prevailed at the time of the accident. The National Transportation Safety Board determines that the probable cause of this accident was the flight crew’s failure to identify and arrest the helicopter’s descent for undetermined reasons, which resulted in controlled flight into terrain.< Less

The Flight Data Recorder (FDR) recorded that many master cautions activated following the failure of the Rudder Travel Limiter which triggered Electronic Centralized Aircraft Monitoring (ECAM) message of AUTO FLT RUD TRV LIM SYS. The crew tried repeatedly to reset the computers but the autopilot and auto-thrust disengaged and the flight control reverted to Alternate Law. The aircraft stalled and crashed. The investigation showed that the loss of electricity and the RTLU failure were caused by a cracked solder joint. All occupants of the plane were killed in the accident.< Less
