Spacecraft and Launch Vehicle Failure

This book is a 10 week course on the basics of spacecraft and launch vehicle failures. This class was given at NASA MSFC in 2007 and 2008 for engineers and managers on the Ares 1 and V programs. It provides and overview of the subject, examples of past mission and two class exercises (analysis of a real spacecraft failure and a launch vehicle failure).
