Smile Evaluation

Conducting a smile evaluation with each new patient will allow you an opportunity to learn more about the patient’s past experience, their perceptions and give the patient an opportunity to express concerns or dissatisfactions they may have with their mouth. The exchange will also create an opportunity for the patient to get comfortable with you and educate them about the services you provide. Included are questions to ask and a form you can use.
