This health journal holds all of your important health information in one place! There is a page at the front to record your doctor's names, specialties, and contact information. The next page is a blank calendar for you to record your monthly appointments at a glance. The following pages alternate between a daily log for medications and appointments and a lined notes page. The daily log pages have a grid for your daily medications with dosages and a grid to log any doctor's appointments for the day. The pages opposite the log pages are lined notes pages to keep track of important information from the day. Your health journal would be helpful to take along with you to appointments to keep all of your notes and information in one easily referenced place.
