Patient Journal

Patient journal is used for keeping a record of patient demographics, a list of the patient’s doctors, a list of allergies, their insurance information, health issues, surgeries, ER visits or hospitalizations, and any other information that a patient would need to carry with them to the doctor at each appointment.
