How to Write a PCR

-PCR & ACR -
PCR and ACR’s are part of documents that an EMT must provide as a part of the patient care process and later long after the call.
The report you write will become a permanent record in the receiving facility, these might be hospitals, Nursing Homes, Dialysis, and some times the regular doctor offices. Usually there are three part of it,
1. Pink Copy is for the medical facility. Hospital E.R, Patient, N.H, Dialysis, Dr Office, etc
2. Yellow for your base company. Private Ambulances, hospital 911 and/or Municipal services
3. White copy is for the Agency. And this is the most important backup of what you have done with you patient.!
Documentation has an important role to help you remember important facts about the treatment you gave at that moment!
