CareAnyWhere Tapestry

The Tapestry is a living, breathing testament to our commitment to patient-centered care. The goal of the Tapestry is to inspire member organizations and beyond to engage their patients in ways that meet them where they are so that our patients will say: “CareAnyWhere exists to help me live to my fullest, on my terms by providing ubiquitous access to my health information and providers in order to maintain my health where I live, work and play.” This is a booklet of promising ideas and well-developed best practices for delivering care with the right people at the right place and time. The ideas and practices are framed from the patient’s perspective and are meant to help shift the thinking of caregivers to a more obvious patient-centered point of view.
