Executive Summary
Project Sentinel, a covert research operation conducted within Ward 13 of the Ironbark Asylum, experienced a catastrophic systems failure during a scheduled synchronisation trial. The failure precipitated an anomalous environmental phenomenon subsequently designated The Rift Seal, which established an impermeable perimeter around the hospital grounds. The perimeter prevents ingress, egress, and all known forms of external communication. Immediate attempts to neutralise or bypass the phenomenon were unsuccessful. Personnel and civilian patients remain trapped inside the sealed perimeter.

To provide a concise, factual account of events leading to the containment breach, catalogue personnel and material losses, assess immediate operational status, and recommend actions for higher command.
Background
Project Sentinel was authorised as a behavioural stabilisation and perceptual extension research program. The objective was to induce controlled neural resonance among selected subjects to explore enhanced perceptual and predictive capabilities. The experiment utilised three primary systems: Neural Resonance Induction (NRI), Sensory Deprivation/Overload Cycling (SDOC), and the Quantum Pulse Stimulus Array (QPSA). Ward 13 was designated as the test environment due to the concentration of selected subjects.
